A COPY OF THE BIRTH CERTIFICATE IS REQUIRED AT REGISTRATION. REGISTRATION FORMS WILL NOT BE ACCEPTED WITHOUT A COPY.

FEE IS $100 PER PLAYER; PRICE INCLUDES UNIFORMS AND TROPHY (CHECKS PAYABLE TO SPOUT SPRINGS SEMINOLES SPORTS, INC).

OPITIONAL INSURANCE IS AVAILABLE AT $6 PER PLAYER. CHECKS SHOULD BE PAYBLE TO RECREATION YOUTH SPORTS (RYS)

INSTRUCTIONAL LEAGUE WILL BE $65 PER PLAYER; PRICE INCLUDES UNIFORMS AND TROPHY
